First: Rent the machine for time period needed
We rent the machine for a minimum of 2 weeks @ $50 or monthly @$75
Second: Purchase the type of pad you need
We sell pads designeqd for ankle, hip, shoulder, knee, or universal.
Third: Follow the instructions on the setup of your theraapy
Follow the setup of the equipment as required and then treat as to a physician's order as to time for each treatment.

- This agreement is for the personal use of mobility equipment offered by Upstate Mobility LLC, located at 2801 Wade Hampton Blvd, suite 301, SC 29687. The equipment is intended to be for personal use and must be used only in the manner intended by the manufacturer.
- The rental unit is not to be used by children and Upstate Mobility LLC will assume no liability for unauthorized use. At no time should the operator allow riders on the unit.
- This agreement is between Upstate Mobility LLC and the undersigned. It is governed by South Carolina laws and is subject to the terms described below. The signer is required to provide a current Driver's license and prepay the amount indicated.
- The terms and rates of the agreement are set at the time of rental, any extension of days will be due prior to the extension.
- Any excessive damage to the rental equipment will be charged to whomever is listed on the rental agreement.
- If you experience any issues with the rental equipment you must contact us for support. Failure to contact us for support will result in the denial of any refund requests at the time of equipment return.
